San Remo Open Access - Application Form

This form is for registering your details to become an Open Access member at the San Remo Library. Once submitted, your application will be reviewed by a Myli team member. If you are eligible, we’ll be in touch to arrange an in-person induction session. A one-off $30 administration fee applies and is payable at your induction session. After completing your induction, you’ll receive your personal Access Fob, giving you entry to your selected library.

Acceptable ID cards

*Please note that one must be a photo ID. Please bring your ID with you to your induction session. If you're applying for a Family Membership, only the adult named in the application needs to provide identification.

Emergency Contact

An emergency contact is required in case we need to reach someone on your behalf while you are using the library outside of staffed hours.

Terms and Conditions

The following terms and conditions apply to using the after hours access service at nominated Myli – My Community Library Ltd (Myli) branches, outside their normal opening hours. These conditions are to ensure the safety and security of everyone using our libraries.

The applicant is required to use all Myli facilities in accordance with the following terms and conditions. Myli reserves the right to accept or reject any application at its discretion and may disregard any requests that fall outside the terms of this agreement.

Our Child Safety Commitment

Myli – My Community Library Ltd is committed to the safety and wellbeing of all children and young people.

1. Access and Membership Responsibilities

1.1. Upon approval of your 24/7 and/or Open Access application form and completion of a building and safety induction, your access fob will be activated and ready to use. If you have trouble using your access fob, please consult a staff or email facilities@myli.org.au. The access fob remains the property of Myli and must be returned upon request.

1.2. You must notify Myli immediately if your access fob is lost or stolen.

1.3. You are responsible for all use of your access fob, including any unauthorised use. Misuse may result in cancellation of access privileges.

1.4. Your access fob is for your personal use only, or for listed family members under an approved family membership. Do not share your fob with others. All individuals must swipe their own fob to enter. Tailgating is strictly prohibited.

1.5. A family membership includes one adult. A second parent/guardian must apply separately for a Family Membership and pay the $30 administration fee. Children can be listed on both accounts and may attend with either registered parent/guardian.

1.6. Children must be supervised at all times. Individuals under 18 are not permitted in the library after hours unless accompanied by a registered adult.

1.7. If you are in the library when it closes to non-members and staff are leaving, you must exit and re-enter using your fob to ensure your access is properly logged.

2. Behaviour and Facility Use

2.1. All Myli 24/7 and Open Access locations are monitored by CCTV. You may be recorded while onsite.

2.2. Please respect fellow library users and staff at all times.

2.3. Respect library spaces and collections. Keep the environment clean and tidy. Dispose of any rubbish appropriately and do not leave food or drink containers behind.

2.4. Alcohol is not permitted. Smoking is prohibited within Myli buildings and within ten (10) metres of the entrance.

3. Borrowing and Returning Items

3.1. All items must be borrowed using the self-service kiosk.

3.2. If the kiosk is unavailable, record your library membership number and item barcodes on the provided paper form. Staff will process loans later.

3.3. Return all items via the designated returns chute. Do not leave returned materials on desks or counters.

4. Safety and Emergency Procedures

4.1. Emergency evacuation procedures and floor plans are posted throughout the library. Familiarise yourself with the nearest exits during your induction.

4.2. Duress devices are available after hours and monitored 24/7. Locate them during your induction. If you feel unsafe or are in an emergency, press the button to alert security.

4.3. To exit after hours, press the red or green door-release button. Ensure the door closes securely behind you.

4.4. If the door does not open or in an emergency, press the BREAK GLASS button. This will trigger an alarm and notify security. Use only in genuine emergencies.

Key Fob Access

The applicant must be an existing MYLI member, a member of Libraries Victoria or join as a new member. The applicant agrees to pay the administration fee of $30 to activate the key fob access. This is not refundable. If the fob is lost or stolen, the applicant agrees to notify MYLI as soon as practicable. An additional cost of $30 is incurred for a replacement fob.
